South Sefton primary care network (PCN) medicines management team has been crowned Pharmacy Team of the Year 2025.
This integrated team of pharmacists and pharmacy technicians are working to improve medicines safety and support patients throughout their health journey, across South Sefton PCN.

They were presented with the coveted accolade at the 17th General Practice Awards, held last week at Novotel London West, to celebrate those who are going above and beyond in primary care.
The awards ceremony was attended by 650 guests and hosted by comedian Suzi Ruffel and voice actor Roger Tilling.
Jenny Johnston, head of medicines management at Sefton Place, told The Pharmacist: ‘It’s a bit overwhelming to be honest with you. We didn’t expect this at all but it’s a real reflection of the hard work the team puts in on a day to day basis. We’re quite an innovative team and we have patients at the heart of everything we do.’
South Sefton PCN medicines management team operates a model that brings together clinical pharmacists and pharmacy technicians working across 19 GP practices. At the heart of the model is a medicines management hub, providing a single point of contact for out-of-stock medicines, clinical queries and safety alerts.

The team also provides pharmacist-led menopause clinics, an adverse childhood experiences health check programme, and medication reviews and personalised care plans for care home residents.
‘There are plenty of plans in the pipeline,’ added Ms Johnston.
‘We’ve got an ongoing project at the moment supporting patients who’ve had acute kidney injuries when they come out of hospital. We visit the patients at home and educate them about what’s gone on in the hospital then we can forward them on to the pharmacists we have in GP practices.’

The five other pharmacists and pharmacy teams shortlisted for the award were Willows Health Pharmacy Team, Leicester; James Wigg Group practice pharmacy team, London; Penshurst Gardens Surgery, Edgware; specialist respiratory pharmacist Nazir Hussain, West Midlands; and PCN Clinical Pharmacy Team, Local Primary Care GP Federation, Blackburn with Darwen.
