Pack sizes for Xarelto (Rivaroxaban) 1mg/ml granules for oral suspension have changed to reflect final reconstituted volumes rather than the previously listed volumes of water required for preparation.

Effective since 1 July 2025, for children weighing 4kg or more, the licensed pack size is now 103.4ml, previously listed as a 100ml bottle.
For children weighing less than 4kg, the new pack size is 51.7ml, previously listed as a 50ml bottle.
The changes follow identification of discrepancies between the described and actual reconstituted volumes, by Community Pharmacy England (CPE).

Previously, the pack sizes listed in the Drug Tariff and the NHS dictionary of medicines and devices represented the volumes of water added for reconstitution and not the final reconstituted volumes of the suspension after reconstitution.
Pharmacies may still receive prescriptions specifying the older pack sizes until prescribing systems are fully updated.
Since both pack sizes are classified as special containers, pharmacies should supply the nearest complete pack size to the prescribed quantity.
