2025/26 CPAF screening questionnaire now open for completion

Pharmacy owners in England have been asked to complete a mandatory self-assessment questionnaire as part of the 2025/26 Community Pharmacy Assurance Framework (CPAF) process.

The CPAF screening questionnaire opened on 1 July and is available to complete in the Manage Your Service (MYS) portal until 11:59pm on Thursday 31 July 2025.

Completion of the CPAF screening questionnaire is a mandatory requirement under the Terms of Service, following NHS regulatory changes introduced in late 2020.

This initial questionnaire is described by the NHS Business Services Authority (NHSBSA) as a ‘short self-assessment tool to help us identify which pharmacies we will ask to provide further assurance by completing the full CPAF questionnaire’.

Information collected will also be used to decide whether to arrange a contract monitoring visit, it said.

All pharmacy contractors must complete the initial screening stage and, where selected, the full CPAF questionnaire.

The screening consists of 10 questions and must be completed via the NHSBSA MYS portal. Contractors not yet registered with MYS will be able to complete the questionnaire through an alternative online form.

The questions are also available on the NHSBSA website to allow pharmacy owners to review them in advance.

Following the screening phase, NHS England will select a subset of pharmacies for further scrutiny, either a monitoring visit or a requirement to complete the full CPAF questionnaire.

Community pharmacies should receive access details and instructions via email from NHSBSA or through their head office.
